Intradialytic versus home based exercise training in hemodialysis patients: a randomised controlled trial
نویسندگان
چکیده
BACKGROUND Exercise training in hemodialysis patients improves fitness, physical function, quality of life and markers of cardiovascular disease such as arterial stiffness. The majority of trials investigating this area have used supervised exercise training during dialysis (intradialytic), which may not be feasible for some renal units. The aim of this trial is to compare the effects of supervised intradialytic with unsupervised home-based exercise training on physical function and arterial stiffness. METHODS AND DESIGN This is a randomised, controlled clinical trial. A total of 72 hemodialysis patients will be randomised to receive either six months of intradialytic exercise training, home-based exercise training or usual care. Intradialytic patients will undergo three training sessions per week on a cycle ergometer and home-based patients will be provided with a walking program to achieve the same weekly physical activity. Primary outcome measures are six-minute walk distance (6 MWD) and aortic pulse wave velocity (PWV). Secondary outcome measures include augmentation index, peripheral and central blood pressures, physical activity and self-reported health. Measures will be made at baseline, three and six months. DISCUSSION The results of this study will help determine the efficacy of home-based exercise training in hemodialysis patients. This may assist in developing exercise guidelines specific for these patients. TRIAL REGISTRATION ACTRN12608000247370

The impact of an exercise physiologist coordinated resistance exercise program on the physical function of people receiving hemodialysis: a stepped wedge randomised control study
BACKGROUND Exercise during hemodialysis treatments improves physical function, markers of cardiovascular disease and quality of life. However, exercise programs are not a part of standard therapy in the vast majority of hemodialysis clinics internationally. Hemodialysis unit-based accredited exercise physiologists may contribute to an increased intradialytic exercise uptake and improved physica...

Background and Objective: Increased homocysteine levels resulting from renal failure and physical inactivity in hemodialysis patients can put them at risk for cardiovascular diseases. On the other hand, exercise training may have a favorable effect on homocysteine levels and physical performance in these patients. Therefore, the aim of the present study was to investigate the effect of eight we...
متن کاملThe effect of citrate dialysate on intradialytic heparin dose in haemodialysis patients: study design of a randomised controlled trial
BACKGROUND Unfractionated heparin is the most common anticoagulant used in haemodialysis (HD), although it has many potential adverse effects. Citrate dialysate (CD) has an anticoagulant effect which may allow reduction in cumulative heparin dose (CHD) compared to standard acetate dialysate (AD). METHODS This double-blinded, randomised, cross-over trial of chronic haemodialysis patients deter...

Background and purpose: Patients undergoing hemodialysis, regardless of age, suffer from impaired physical function. Many problems caused by lack of mobility affect different dimensions of their lives. Exercise training, commensurate with the ability and willingness of hemodialysis patients could be of great benefit in dealing with the lack of ph...
متن کاملThe effect of intradialytic aerobic exercise on dialysis efficacy in hemodialysis patients: a randomized controlled trial.
OBJECTIVE This study was conducted to determine the impact of an 8-week intradialytic exercise program (consisting of 15 minutes low-intensity exercise during the first 2 hours of dialysis) on dialysis efficacy. METHODS In an open randomized controlled trial, a total of 50 clinically stable hemodialysis patients were enrolled into the study and randomly allocated into two groups: the aerobic ...
